Eligibility for Services
Acumen Residential Services Corporation provides non-skilled residential and community-based supports to adults who qualify under approved programs.
Eligibility is determined through the appropriate state and county agencies and may depend on factors such as:
- Participation in an approved Medicaid waiver program
- Assessment of individual support needs
- Availability of services within the service area
Final eligibility decisions are made by the relevant funding and oversight agencies.
How Referrals Work
Referrals to Acumen may be initiated by:
- Case managers
- Support coordinators
- State or county agencies
- Families or legal representatives
All referrals are reviewed to ensure alignment with Acumen's service scope, values, and capacity.
After a Referral Is Submitted
Once a referral is received:
- Our team reviews the referral information
- We assess service compatibility and availability
- Additional documentation may be requested
- Next steps are communicated clearly to all parties
