Acumen provides non-skilled non-medical residential and community-based supports designed to promote independence, safety, and quality of life. We do not provide skilled nursing, medical, or clinical services.
All services are delivered in accordance with individualized service plans and applicable Medicaid waiver requirements.
Residential & Community Living Supports
We support adults in maintaining stable living environments while building daily living skills.
Services may include:
- Assistance with daily routines
- Support with budgeting and household management
- Community participation and engagement
- Skill-building for independent living
Day Programs
Our day programs offer structured, meaningful activities that promote:

Personal Support

Acumen participates in approved Medicaid waiver programs, delivering services aligned with person-centered plans and regulatory requirements.
Eligibility and service availability are determined through appropriate state and program processes.
